What Causes Neck Pain?

Neck pain has many causes. An irritation in your cervical spine can produce symptoms anywhere from your head and shoulders down to your arms, wrists, and hands. At SPIN, we diagnose and treat the full spectrum of neck pain conditions - from everyday muscle strain to complex nerve compression.

Common causes of neck pain we see at our Manhattan clinic include:

Poor posture and tech neck

Poor posture and tech neck

long hours looking down at a phone or screen strain the cervical muscles and joints

Herniated or bulging discs

Herniated or bulging discs

a disc in the cervical spine that presses on nearby nerves, causing sharp or radiating pain

Whiplash

Whiplash

soft tissue injury from a sudden backward-forward motion, common in car accidents and contact sports

Cervical radiculopathy

Cervical radiculopathy

a pinched nerve root in the cervical spine causing pain, tingling, or weakness radiating into the arm

Cervical spondylosis

Cervical spondylosis

age-related degeneration of the cervical spine joints and discs

Cervical spinal stenosis

Cervical spinal stenosis

narrowing of the spinal canal putting pressure on the spinal cord or nerve roots

Muscle spasms and strains

Muscle spasms and strains

overuse, stress, or sudden movements that tighten or tear soft tissue

Arthritis

Arthritis

wear-and-tear or inflammatory arthritis of the cervical facet joints

Chronic neck pain may also develop from spinal cord deformities, tumors, or untreated acute injuries. Determining the exact cause requires a thorough history, physical examination, and often imaging - which is exactly what Dr. Melepura provides at your first visit.

Symptoms That Tell Us Where Your Neck Pain Is Coming From

Our team relies on your testimony about your pain in the neck to make a diagnosis. You need to convey clearly:

  • When the pain started
  • What you were doing when it began
  • When it's most painful
  • What kinds of activities provide temporary pain relief

Make sure you tell our pain specialists about any:

Stiffness in your neck

Difficulty turning your head

Sharp, shooting pains

Pain that radiates upward to your head

Radiating pain down your arms

Tingling in your arms, hands, or fingers

Tenderness in your neck

Difficulty holding objects in one hand or the other

Loss of total bladder control

Balance, walking, or coordination problems

Headaches

Types of Neck Pain We Treat

Not all neck pain is the same. Here are some of the most common causes we see in our office:

Tech neck

Looking down at your phone or computer for hours strains the muscles in your neck. This is one of the most frequent causes of daily neck stiffness and soreness.

Poor posture

Slouching at your desk, driving with your head pushed forward, or even sleeping without the right pillow can all lead to chronic muscle tension.

Herniated disc

A disc in your cervical spine can bulge or slip, pressing on nearby nerves. This often causes sharp pain that may shoot into your shoulder or arm.

Whiplash and injuries

Car accidents, sports injuries, or sudden falls can stretch or tear soft tissue in the neck. You may not feel the pain right away, but it often worsens over the next few days.

Arthritis

Wear-and-tear over time can break down the joints in your neck, leading to stiffness, aching, and reduced range of motion — especially in the morning.

Pinched nerve

When surrounding tissue puts pressure on a nerve, it can cause pain, tingling, or weakness that travels down your arm or into your hand.

If your neck pain is lasting more than a few days or interfering with sleep, driving, or your daily routine, don't wait it out. We'll help you get to the root of it and create a treatment plan that actually works for your lifestyle.

How We Diagnose Your Neck Pain

Once your pain management doctor hears your history and symptoms, he performs a physical examination to determine how to treat neck pain in your individual circumstance.

Dr. Melepura will observe how you move, sit and hold your head. He feels along your cervical spine for muscle tightness that could indicate spasms or damage to the soft tissue. He does a few range-of-motion tests for further insight into how to get rid of neck pain for you.

The first round of treatment involves home care, but if that initial treatment doesn't do the trick, our doctors may need further testing that could consist of:

  • X-rays
  • Myelogram
  • CT Scan
  • MRI
  • Bone scan
  • Nerve conduction study
  • Blood tests

Neck Pain Treatments We Offer at SPIN

Most neck pain treatment consists of rest and a few simple lifestyle changes.

At your center, you also may receive instructions for:

Massage
Ice or heat therapy
Improving your posture
Work modifications
Over-the-counter anti-inflammatory pain medicine

If your pain in the neck doesn't respond within three to four weeks, you may need a little more aggressive neck pain relief.

When conservative care is not enough, Dr. Melepura may recommend one or more of the following:

Cervical epidural steroid injection

delivers anti-inflammatory medication into the epidural space around the cervical nerve roots; most effective for disc herniation and radiculopathy

Cervical facet injections

targets the small joints of the cervical spine; used for axial neck pain and cervicogenic headaches

Medial branch nerve block

diagnostic injection that confirms cervical facet joints as the pain source before radiofrequency ablation

Radiofrequency ablation

uses heat to interrupt pain signals from cervical facet joint nerves; provides relief lasting 12 months or longer

Trigger point injections

releases tight muscle knots in the neck and upper back contributing to chronic pain

Occipital nerve block

treats cervicogenic headaches and occipital neuralgia originating in the upper cervical spine

Nerve block injection

interrupts pain signals from specific cervical nerves causing radiating arm pain

Physical therapy

prescribed alongside procedures to restore range of motion and prevent recurrence

When Is Surgery Considered for Neck Pain?

Surgery is considered only when all conservative treatments and minimally invasive procedures have failed to provide adequate relief. At SPIN, we exhaust every non-surgical option before discussing surgical referral.

Stabilize your neck
Take out a damaged disc
Remove a nerve root
Release tension caused by compressed discs

Your doctor provides you with all the information you need to make an informed decision about how to treat neck pain. Returning you to your normal activities, sports, and fitness routines, pain-free, is always the goal at our pain treatment center.
